To officially get started, please complete the short onboarding form as well as the client agreement below. Once those are submitted, you’ll be able to choose your payment option, complete your transaction, and officially reserve your space.

Step 1: Set Your Intentions.

Step 2: Sign and complete your client agreement.

Note: To utilize the online Google Form below, you will be required to sign into your personal gmail account. If you complete the Google Form you will not need to email the PDF.

How to Complete PDF client agreement:

  1. Open the PDF using Adobe Acrobat Reader (free) or your browser.

  2. Fill in the required fields:

    • Client Name

    • Email

    • Signature (you may type your name)

    • Date

    • Check the box confirming you agree to the terms.

  3. Save the completed file.

How to Return:

  • Email hello@denisefaddis.com and attach the filled-in PDF.

  • Please aim to return it within 48 hours so we can move forward smoothly.
